Woodlands pride’s itself on the care-planning system we use. The plans are updated on at least a six monthly basis but in reality they are reviewed constantly. We also have a health action plan in place (which is part of the care-plan) which is taken to all medical appointments and gives a current history of each individual’s health.

When an individual comes to live at Woodlands a comprehensive pre-admission assessment is completed, (bringing in professionals if needed) the full care-plan can take several months to put into place as we get to know individuals.

Services enjoyed by the service users:

PERSONAL CARE
We cater for all levels of personal care; individuals that need 100% care to residents who only need verbal prompts.

We strive to find the balance between promoting independence and choice with safety issues. Each individual resident is assessed according to their needs, wishes and abilities. The resident, their family and other professionals are consulted and pervious history is considered. We work hard towards minimising the likelihood of risk a risk occurring and safe-guarding any risk that might be identified.
