ADMISSION CRITERIA
Ashwood is funded by Social Services to provide care/support for service users, via Social Service level agreement.
Within Ashwood service users will be given a service users guide, which has been updated to ensure the service users find it user friendly.
COMMUNITY CARE ASSESSMENTS – it is essential that the home has a detailed Community Care Assessment to be able to assess whether the service the home provides can meet the prospective service users needs and that he/she will be compatible with the current service user group. The individuals Service Users Plan is formulated using their Community Care Assessment as a basis.
For a privately funded placement the detailed assessment will be carried out by the home and the Service User Plan will be formulated using this Assessment as a basis.
- PLANNED ADMISSIONS - In the event of a vacancy the following procedure will be adopted:
REFERRAL:
A referral is made from Social Services or any outside agency.
STAGE 1
As soon as possible after the referral date an initial visit will take place by the Home Manager to meet the prospective service user and complete the Community Care Assessment form with Social Worker, current carer or representative. This visit will take place at the person’s current residence or at some accessed day provision. Further meetings may need to be made by agreement with the prospective service user and the Learning Disability Team. A Community Care Assessment will be required from the placing authority prior to admission.
STAGE 2
The home will organise for the individual, their families, advocates etc. to visit the home. Visits in the first instance are usually invitations for tea, this may happen several times, during these visits the prospective service user will be able to meet other service users and staff, look around the home and view their room. An overnight stay followed by a weekend stay is then organized (if appropriate). The introduction is done at the person’s speed; it may take a longer or shorter period of time depending on each individual.
STAGE 3
If there are no objections to the referral a formal written offer will be made to the individual or their representative, the Service Users Contract is completed and signed and a review date is set.

Care Home Manager to meet the prospective service user and complete an Assessment with the Social Worker, current carer or representative. This visit will take place at a mutually agreed location. A Care Assessment is required from the placing authority, if possible this should be completed prior to admission, if this is not possible as much detailed information as possible should be provided by the placing authority, the Care Assessment should then be completed within 5 working days
STAGE 2
A formal written offer will be made to the individual or their representative, the Service Users Contract is completed and signed and a review date is set.
